Specimen
Mycobacterium avium complex isolated from a primary clinical specimen
Fasting
Not stated
LOINC

Turnaround

Laboratory assay overview

Determine the susceptibility of Mycobacterium avium complex isolates to a profile of antimycobacterial agents. Testing is recommended for isolates from patients with previous macrolide therapy, patients who relapse while on macrolide therapy, or patients with initial isolates from blood, tissue, or clinically relevant respiratory specimens. Repeat susceptibility is recommended after 3 months (disseminated disease) or 6 months (chronic pulmonary disease) if the cultures are still positive or if there is no clinical improvement. First line drugs for M. avium complex treatment includes clarithromycin and amikacin (IV or inhaled). Clarithromycin is the most appropriate class drug for the macrolides due to technical challenges with azithromycin. Second line drugs include moxifloxacin and linezolid, however, the in vivo effectiveness of these drugs for treatment of M. avium complex is unproven. These are the only drugs with CLSI interpretations for M. avium complex. Note that while ethambutol can be used as part of the treatment regimen for M. avium complex, studies have shown that MIC values do not correlate with clinical outcomes. For this reason, ethambutol is not reported for M. avium complex isolates.

Mycobacterium marinum Susceptibility (AST)−Broth Dilution

Laboratory
Labcorp
Test code
182840
Specimen
Mycobacterium marinum isolate from a primary clinical specimen should be submitted on conventional solid or broth AFB broth medium.
Fasting
Not stated
LOINC
29576-6
Turnaround
21 - 28 days

Laboratory assay overview

Determine the susceptibility of Mycobacterium marinum isolates to relevant antimycobacterial agents. Routine susceptibility testing of M. marinum isolates is generally not recommended.1,2 The following agents are typically found to be susceptible when testing M. marinum: Rifampin, Ethambutol, Doxycycline, Minocycline, Clarithromycin and Trimethoprim-sulfamethoxazole.3 Disease due to M. marinum is generally localized and the number of organisms is low, with 95% of tissue biopsies reported as AFB smear negative. Susceptibility testing should be considered when patients remain culture positive after several months of therapy.4

Laboratory
Labcorp
Test code
182923
Specimen
Slowly-growing mycobacteria
Fasting
Not stated
LOINC
29579-0
Turnaround
35 - 40 days

Laboratory assay overview

Determine the susceptibility of nontuberculous slowly-growing mycobacterial isolates to a profile of antimycobacterial agents. Clinical breakpoints for M. kansasii and Nontuberculosis Mycobacteria other than M. avium Complex are the same. Some fastidious isolates may not exhibit adequate growth for susceptibility testing ( M. genvense, M. haemophilum ). Others such as M. gordonae are rarely clinically relevant and susceptibility testing is not recommended. If patients are not responding to several months of therapy, repeat susceptibility testing is recommended.

Laboratory assay overview

Susceptibility reports can be used as a guide in the choice of first course therapy or in confirming antimicrobial resistance that leads to a further course of treatment. Also used to estimate the prevalence of primary and acquired antimicrobial resistance in a community.

We compile every detail above directly from named laboratory directories and published clinical studies. Laboratories calculate standard reference intervals from the statistical distribution of tested populations for a specific assay method and demographic group, rather than setting universal optimal targets.
